This webpage provides the dates and instructions needed by students and alumni who wish to have a committee letter prepared by HPAC on their behalf. Pay careful attention to the deadline dates and requirements detailed below. If you miss a deadline or your file is incomplete, the HPAC will not write a Letter of Evaluation for you.

Deadline to Request a Committee Letter for MD, DO, DMD Programs

STUDENT MUST SUBMIT:

  1. An official request: Send an email that will serve as your official request for a Letter of Evaluation to Professor Alison Draper via e-mail at [email protected] (cc’ed to Kathy Mallinson)
  2. All necessary signed letterhead forms: Sign and email/mail/hand-deliver the HPAC Letter of Evaluation form to Kathy Mallinson (Clement 107, Science Commons).
  3.  Personal Statement – send as email attachment (Word or PDF files, please) to Kathy Mallinson. Guidelines: Personal Statement
  4. Autobiographical Profile – send as email attachment (Word or PDF files, please) to Kathy Mallinson. Guidelines: Autobiographical profile guidelines
  5. Send Kathy Mallinson a list of the people who will be writing letters of recommendation for you. This is also the deadline to request Letters of Recommendation (minimum of 3). See further guidance below. Attached here is a guide you may share with anyone writing a letter of recommendation on your behalf: Guidelines for Letters of Recommendation Faculty Writers

The letterhead forms can be found here:

  • MD programs: HPAC_Committee_Letterhead -AMCAS
  • DO programs: HPAC_Committee_Letterhead – AACOMAS
  • Dental school: HPAC_Committee_Letterhead – ADEA
  • TYPE your Name, Class Year and Professional School or Program (Allopathic Medical School, Osteopathic Medical School, or Dental School) in the correct lines on the form.  Your letter will be printed on this form, so be sure that this is done exceptionally neatly.
  • Indicate whether you wish the Committee Letter to be Confidential or Non-Confidential by signing your name and writing the date in the appropriate box. (Professional schools generally expect a confidential letter).

Requesting letters of recommendation earlier is better.  Two letters MUST be from Trinity SCIENCE faculty and one letter MUST be from a NON-SCIENCE faculty. (These are expectations for many programs.) One or two additional letters may be submitted at the students discretion (athletic coaches, job supervisors, research mentors, etc.).

Letter writers will have until April 6 to submit their letters to Kathy Mallinson ([email protected]). Letters should be signed and on letterhead.

IMPORTANT: It is your responsibility to check the status and content of your file by contacting Kathy Mallinson and to remind writers of your deadline. Writers must send recommendations directly to Kathy Mallinson as a pdf file. Letters must be on the official institutional letterhead and must have an original signature by the letter writer. Letters not on letterhead or signed will be returned to the letter writer.
