Just a reminder that Trinity employees are required to submit their COVID 19 vaccination verification cards by this Friday, July 23. Exemption requests (process communicated on 7/7/21) must be submitted by Wednesday, July 21.
To submit your vaccination information please follow these instructions:
- Take a picture of your completed vaccine card so it is ready for upload.
- Visit https://trinity.studenthealthportal.com.
- Sign in with your Trinity credentials.
- Choose “My Forms” in the navigation bar.
- Choose the “Covid 19 Vaccination form”.
- Type in the dates of your vaccines in the row that corresponds with the manufacturer of the vaccine you received.
- Upload a photo of your completed vaccine card by choosing the “upload” box.
Health Center staff will email you if there are any questions related to your documentation.

Employees should continue to monitor their health and seek healthcare advice from your primary care provider if you are ill or concerned you may have developed COVID illness.
