Dear Colleagues,<\/p>\n

I wanted to share a few updates\/reminders for covid testing and booster requirements.<\/p>\n

Covid Testing and Contact Information in Patient Portal<\/h4>\n

Before your next test, please review and update your contact information in the lab\u2019s patient portal. After logging in<\/a>, click the \u201cProfile\u201d tab to update your details.\u00a0 Please include a contact number where you can be reached after normal business hours.<\/b>\u00a0 This number will only be used to communicate a positive result. As a reminder, results are available through CareEvolve, and this system uses a different username and password from your standard Trinity credentials. You can reset your password by following the forgot password link on the log-in page.<\/p>\n

Employees who have symptoms, are currently positive, or who have tested positive within 90 days should not come to the center to be tested.\u00a0 If you have symptoms, please obtain a test off campus and contact your health care provider.\u00a0 Positive results obtained off-campus should be communicated to David.Deresienski@trincoll.edu<\/a> in Human Resources.<\/p>\n

Booster Shot Documentation<\/h4>\n

Verification of booster shots per the January 13 email are required by February 1, 2022.\u00a0 If you have not done so, please email proof of your booster to Kathleen.Durante@trincoll.edu<\/a> as soon as possible.<\/p>\n

You will still need to submit proof if you receive the booster at a Trinity clinic.<\/p>\n

Information on the Booster Clinics is attached here<\/a>.\u00a0 We are currently determining whether to schedule more clinics later in February and will communicate details if\/when they are confirmed.<\/p>\n
