{"id":1204,"date":"2021-07-07T13:49:07","date_gmt":"2021-07-07T13:49:07","guid":{"rendered":"https:\/\/www.trincoll.edu\/reopening\/?page_id=1204"},"modified":"2021-07-07T13:49:07","modified_gmt":"2021-07-07T13:49:07","slug":"vaccination-exemption-process","status":"publish","type":"page","link":"https:\/\/www.trincoll.edu\/reopening\/vaccination-exemption-process\/","title":{"rendered":"COVID-19 Vaccination Exemption Request Process"},"content":{"rendered":"<p>July 7, 2021<\/p>\n<p>Dear Colleagues,<\/p>\n<p>As you know, Trinity College will require all faculty and staff to be fully-vaccinated against COVID-19 by August 2, 2021, unless they have been approved for an exemption for medical reasons or a sincerely held religious belief.\u00a0 I\u2019m writing today to share the process to request an exemption.<\/p>\n<p><strong>Exemption Request Process<\/strong><\/p>\n<ul>\n<li>Exemption request forms are located on the <a href=\"https:\/\/trincoll.sharepoint.com\/sites\/HR\/CurrentInfo\/default.aspx\">HR Share Point site<\/a>. Click on medical exemption request form or religious exemption request form.\u00a0 Note you may need to be logged into the network to access the SharePoint and these forms.<\/li>\n<li>Complete all required information on the form.\u00a0 For medical exemptions this includes requesting that your medical provider completes and certifies section 2 on the medical exemption request form and returns the form to Human Resources.<\/li>\n<li>Send completed forms to <a href=\"mailto:human-resources@trincoll.edu\">human-resources@trincoll.edu<\/a> or Trinity College Human Resources, 300 Summit Street, Hartford CT\u00a0 06106.<\/li>\n<li>All forms will be kept confidential.<\/li>\n<li>A health care provider designated by the Health Center staff will review medical exemption requests. Human Resources will review religious exemption requests and will work with employees for medical and religious accommodations.<\/li>\n<li>Employees will be notified as to whether the exemption is approved, not approved, or more information is required.
