Instructions for Completing Forms
for Institutional Review Board (IRB)

The Institutional Review Board (IRB) review campus research projects that involve the collection of information from human participants. All research conducted by students and faculty for class projects, independent research, or extra-curricular activities, whether on- or off-campus, must be reviewed.

Complete all sections of the form "Request for Project Review" and send a copy of the completed form and all supporting documents (e.g., questionnaires, informed consent documents) to the Chair of the IRB. The IRB Chair will determine whether the project will be reviewed under expedited procedures or by the full IRB. In the case of a full IRB review, you will be asked to provide additional copies of all required materials.

Timing: Expedited reviews generally will be completed in one week after application is received. A full IRB review may take up to four weeks. IRB approval is valid for up to one year for faculty research projects; student projects will be approved for the academic year. Shorter approval periods may be granted when warranted by potential risks of the project. Projects of longer duration must be reviewed and approved by the IRB each year.

All members of the Trinity community conducting research at other institutions must submit a copy of the research protocol and a letter from that institution's IRB approving the research. The IRB at Trinity will issue a letter acknowledging the receipt of the approval from the external IRB.

Contact the Chair or any other member of the IRB if you have questions about review procedures. Board members can answer questions about confidentiality or informed consent issues or about risks to human subjects. In addition, a copy of Protection of Human Subjects (Code of Federal Regulations 45 CFR 46) and a copy of the American Psychological Association’s Ethical Principles in the Conduct of Research with Human Subjects are on reserve in the library.
