All new student-athletes to Trinity College please read the following information and complete the forms located at the bottom of the page.

 

Sports Participation & Medical Clearance of Student-Athletes

 

Health Services and the Sports Medicine Department must screen all student-athletes in order to participate in intercollegiate athletics.  All requirements must be completed fully before the student-athletes may practice.  Athletes will not be partially cleared by either Health Service or the Sports Medicine Department. 

 

 Medical Requirements

 

Pre-Entrance Physicals

All freshman, transfer and first year student-athletes must obtain a physical from his/her personal physician prior to entering the college.  This form must be on file at the Trinity College Health Center prior to participation in any sports related activity.

Make sure your physician signs the section that states you are cleared to play intercollegiate athletics.  The examination must be administered within six months prior to participation in any practice, competition, or team conditioning activities. If you did not receive these forms, contact the health center at 860-297-2018.

 

Pre-Participation / Medical History Questionnaire

Student-athletes have the responsibility to truthfully and fully disclose their medical history and report any changes in their health to the team Athletic Trainer. A questionnaire must be filled out each season along with a general health and orthopedic evaluation performed in the athletic training facility.  The procedure includes previous injuries screening, height, weight, blood pressure and pulse.  The team physician and/or college physician or their designee is the final authority concerning when the student-athlete may return to partial or full participation following a previous injury or illness.

 

Insurance Information Form

Each year a new insurance information form must be filled out and on file in the sports medicine facility in order to be eligible for practicing or conditioning.

 

 Directions to Complete Forms (Please use a PC instead of a Mac)

 

1. All Forms should be returned via email prior to July 15, 2008

 

2. Please complete and save the following forms to your computer.

3. Follow the instructions on each form to complete all documents.

4. SAVE each document and close it.

5. YOU MUST USE YOUR TRINITY E-MAIL ACCOUNT FOR THE FOLLOWING

  • attach the completed forms to one email (in the subject line type "NEW", your first and last name, along with your sport; (ex: NEW, jane doe, w. soccer))
  • return to this e-mail address: sportsmedicine@trincoll.edu
