  • Confidentiality of information in your records
  • Receive accurate and understandable information about your health
  • Know the significant beneficial effects, side effects and problems of treatment
  • Participate in choosing a form of treatment
  • Receive education and counseling
  • Consent to or refuse any care or treatment

  • Review your medical record with a clinician
  • Be informed about services and any related costs
  • Receive appropriate referrals to other providers and services
