3 HEALTH REQUIREMENTS for new students:
1) PHYSICAL FORMS (4 pages)
For First-Year Students (class of 2013) - click here
For Transfers - click here for instructions and complete this required physical form
2) HEALTH INSURANCE
All undergraduates, regardless of site of study, MUST have health insurance and provide proof of that coverage EVERY school year. This is done by either enrolling in a student insurance plan offered to students or by waiving the plan by providing your insurance information to us on-line. The only way to remove the student insurance fee of $775 from your tuition bill is by waiving coverage on-line.
Please click here to WAIVE out of the coverage offered or to ENROLL in the coverage offered.
We cannot accept written waivers on tuition bills.
If you have questions, please contact our agent University Health Plans at (800) 437-6448 or info@univhealthplans.com.
3) ALCOHOL Edu on-line exercise
This exercise is required for all incoming students. It is on-line and takes place in 2 parts. The first part MUST be done by the time you move on to campus (Sept 3). The first part takes about 3 hours to complete so please plan your time accordingly to get this done in convenient sessions.
The second part is done 30-45 days after you move in and it takes about 30 minutes. I will send reminder emails to finish the second part.
