Pay careful attention to the deadline dates and requirements detailed below. If you miss a deadline or your Interfolio file is incomplete, you will not be allowed to participate in the HPAC Interview process. The HPAC will not write a Letter of Evaluation for any individual who has not completeed the HPAC Interview process.

The Health Professions Advisory Committee (HPAC) will present information on the materials you need to submit for your HPAC file to qualify for a Letter of Evaluation.

DEADLINE for establishing your HPAC Interfolio Account.
Current Students and Alumni/ae must establish an Interfolio account directly (https://account.interfolio.com/dossier/signup) to which all required materials are submitted.
Obtain the HPAC promotional code at the January 27 meeting or by contacting email@example.com.

Your Interfolio account must contain all of the following materials:
1.) Letters of Reccommendation (minimum of 3) - click on below document for guidelines

Two letters MUST be from Trinity SCIENCE faculty.
One letter MUST be from a NON-SCIENCE faculty.
IMPORTANT: It is your responsibility to check the status and content of your Interfolio account and to remind writers of your deadline. Writers must send recommendations directly to Interfolio.

Sign and deliver two copies of the HPAC Letter of Evaluation form to Kathy Mallinson (CCB 107, Science Commons).
The form can be found here:HPAC_Committee_Letterhead.doc
On both copies of the form, indicate whether you wish the Committee Letter to be Confidential or Non-Confidential by signing your name and writing the date in the appropriate box.
Leave the "Name, Class Year and Professional School or Program" lines blank.
Interview with Prof. Church and assigned HPAC member (this will complete your HPAC Application Folder). These are two separate interviews. Please contact each individual professor ro schedule an interview date and time.

Submit completed primary health professions applications to health professions schools.
Submit a copy of your primary application and letter of evaluation request form (AMCAS application) to Kathy Mallinson (firstname.lastname@example.org) and 'cc' to Dr. William Church (email@example.com).
Submit all secondary applications, fall grades, and summer updates to individual schools.
Prepare for interviews.
2015-2016 Academic Year
Monitor the status of your online Interfolio file. It is your responsibility to ensure that all materials are submitted. Check often and carefully before the deadline.
